X-ray
X-ray patients may walk-in during regular business hours.
Ultrasound and Mammography
Questions we ask physician offices when they call for an appointment:
- The type of exam requested.
- The urgency of the appointment.
- Patient's health number, name and birthdate.
- Name of referring physician.
- The patient’s contact phone numbers.
If mammography:
- When was the last mammogram done and where?
- Is there a concern or is it routine?
- If it is a follow up, is it a 3, 6 or 12-month follow up?
If ultrasound:
- Obstetrical:
- Last menstrual period (LMP)?
- What type of obstetrical: nuchal, 20 week, biophysical profile, etc.?
- All other ultrasounds
- Type of ultrasound?
- If it is a follow up (eg., thyroid or renal), when is it needed?
We will then give the physician's office the appointment date, time and the location where the appointment is booked.
We ask that physician's offices fax the completed requisition to 306-522-4311.
Please remember to provide the patient with any preparation instructions required for the exam. Patient preparation information can be found on this website under 'Services'.
Instructions for Physicians Completing Requisitions
When completing the requisition, physicians are asked to indicate which location that they are working at so that reports can be directed correctly.
Ultrasound Requisitions
- Provide clinical information relevant to the examination being ordered.
- If you are not sure of the examination you wish to order, call us at 306-779-1500 and ask to speak to one of our radiologists.
- Print out the second page and give it to the patient as this sheet contains important preparation instructions.
- Write legibly.
These steps will provide us with important clinical information and will ensure that patients are prepped properly.
Mammography and X-ray Requisitions

Instructions for Patients When Requesting Films
Please phone 306-779-1500 to request previous films.
Have the following information available:
- What procedure was it for? Describe the body part and side, e.g., left or right arm.
- Would you like a print film or a disk (CD)?
- Which physician is requesting the film/CD?
- Is it for personal use? (If so, there will be a charge.)
